Please include a passport size photograph along with your entry form. You should mention your name at the back of the photograph. Please do not staple or stick the photograph.

In case you do not have an email address, please include a self addressed postcard/envelope so that we can send your e-sol-inc. entry no. To avoid delay an email address is preferable.

This is a common entry form for all events. You are requested to fill up a separate entry form for each event. Your entry can be sent by post or delivered to our office.

Parent’s/Guardian’s signature is needed if the participant is a minor.

All documents and photographs sent will be the sole property of e-Sol-Inc. E-Sol-Inc will not be responsible for any loss in transit or damage to the sent items. I/we agree that e-sol-inc’s decision regarding the competition is final and binding upon me/us.
